Can I Exercise?
One barrier to starting an exercise program for many is not knowing if they can. Clearance and encouragement from their doctor may be all they need to get started. 

Assess Physical Activity
Doctors should assess physical activity level at every visit. To begin, use this simple, Prescription and Referral Process. First, determine your patient's current physical activity level. Then, decide if your patient is healthy enough for independent exercise. And finally, if your patient exercises less than the recommended level, use the Stages of Change model to help your patients increase their activity levels.

It's that easy!  But you'll need to be prepared. Get the forms ready and start you assessments today. You'll also want to use the Physical Activity Readiness Questionnaire and provide your patient with a Physical Activity Clearance Form.  These tools will give your patient the confidence they need to get started.
